If you have the fee to apply for one you can do so at any stage.
IVA started March 2011, Completed March 2016 and certificate issued 11 days after final payment. It was not always easy but then some of the best decisions aren't.

You can, but you will need to allow the IVA to fail and get a notice of termination first. If this is problematic the circumstances will have to be explained to the judge.
My opinions are merely that .. opinions based on experience. Always seek professional advice.
IVA Completed 23rd July 2013 .... C.C. 10th January 2014

Yes but if your IVA fails you may find the IP bankrupts you and saves you a job. How long has the IVA been going and what payments are left? Depending on the circumstances you might be able to end the IVA early rather than terminate/breach it...
