Do I have to disclose to current IVA that I been diagnosed with Alzheimer's ?

No -- unless it affects your ability to earn ( work) -- in which case you still don't need to be specific. However, depending upon where you are in the IVA should that time come, creditors aren't ogres.
My opinions are merely that .. opinions based on experience. Always seek professional advice.

IVA final payment left the bank on the 26th January 2013...looking forward to a debt free future.

I see no harm in letting your iva firm know about this but you are certainly under no obligation to do so.

Get the help and support you need first including sorting out any lasting power of attorney.

It may be possible that given the situation creditors accept payments to date but a lot will depend on how supportive your iva company is.
IVA started March 2011, Completed March 2016 and certificate issued 11 days after final payment. It was not always easy but then some of the best decisions aren't.
