i need a copy of the IVA I completed 15 years ago

I had an iva 15 years ago with my ex wife.
I recently applied for ppi the bank wants a copy of the completion of the Iva.
I can’t remember the name of the company we went to to get a copy from.
Is there anyway I can find out or somewhere I can go to for help

Possibly the Insolvency Service or your local court, if either keep records that far back.

I am not sure records go back that far but the insolvency service would be my first port of call.
Random thought - did you use email at all during your iva ? If so search your sent items - I recently did this for something and it was still there 10 years ago.

Ask the Insolvency service to see if they have kept a copy or have details of your ex IP.
