Never asked to sign an RX1 form

I’ve been in an ova for 12 ish months and have just been sent a breach notice that states I haven’t returned a signed RX1 form.
I have never been asked in my 12 month term to fill one in and find it odd that they have just raised the issue.

I would appreciate your thoughts on this

This is not uncommon, but they are being a little heavy handed in correcting what is, after all, their oversight !
I would respond, in writing, direct to the IP, keeping copies, to the effect that it is their error in not requesting that the form be signed, or providing said form for signature. You may want to leave it at that, but I, being a bit snotty like that, would go on to complain at their ommission, compounded by them attempting to shift blame and request an apology, in writing.

My 5 years is up in September. My ex refused to sign the RX1 and they've never questioned it or chased it.
