I have passed the 5 year mark of my IVA and had no contact or formal notice from the company looking after it. The practitioner hasn’t been in touch. We have equity in property and they have a charge on it. What do we do next? Should we have a notification of an extension in place by now? I am continuing to pay each month.

Is it Aperture by any chance ? It is up to you to make sure they act on the equity release clause. Get in touch with them and tell them to pull their finger out.
My opinions are merely that .. opinions based on experience. Always seek professional advice.
IVA Completed 23rd July 2013 .... C.C. 10th January 2014

If you have standard terms then it's up to you to deal with the valuation at month 54. Did you do this?
