My partner has had a pay rise (he is not included in my IVA), and it is time to complete my annual review. How do i show his outgoings have increased too? We share a bank account and he has his own bank account. All our income goes into our joint account, and obviously his bills go out of there too- but how do i declare it on my annual review form?

They don't need to know -- his finances are his concern (unless you are with Payplan and are possibly paying an inflated payment). At the start they will have used his income to work out your proportional "fair share" of the household expenses and they can use this proportional divide again.
My opinions are merely that .. opinions based on experience. Always seek professional advice.
