Can any one tell me:
When filling in bk forms under the bank account section do you have to supply information of bank accounts that you had in the past but are now closed down??

I think you have to list any accounts you have had in the past 2 (I think - check on the forms) years whether they are closed or not.

When you did your forms how did you find out the dates you took out your credit cards/loans?

I had a copy of my credit report - you can get this free online from either Equifax or Experian (sorry, I can't remember which one) - and I used the dates from that.
