Am about to try for a DMP with the charity site, i.e do it myself.

Apart from my ependiture form showing what I can afford to pay etc is there anything else my creditors will expect with my first letter, bank statements etc.

No - I think that creditors will just want so see how much you can afford to pay them on a monthly basis initially. After they have received your letter they may require additional information to be produced, but I would wait and see.
