Cleaning my credit file After completing my iva I had a default from pco holdco which they removed when I sent them my completion documents, but since the 11 th of April they have re added the default, why would they do this equafix said that they sent them a update. It is only on my equifax credit report, it's so annoying please help

I would suggest you re-send them the completion certificate asking why they would do this and ask them to refrain from doing this again. When clearing my own files anther poster did advise me to keep a check as this can happen which has now prompted me to go look.
Hope you get it removed and they refrain from any further defaults.
