Help with documents?

Hi i had a failed IVA back around 2010 . i have been trying to pursue a PPI claim against HSBC for over 8 years and currently have a complaint in place with the financial ombudsman service against HSBC .
HSBC are asking for the following information -
A copy of the IVA proposal, confirming whether it was written on standard terms and the year the terms were written i.e. R3 or 2008, 2010, 2013 or 2014 Protocol terms. This should also include details of the rules governing the IVA and any modifications made.
A copy of the Certificate of Completion for the IVA (If appropriate) .
Confirmation that I am not bankrupt .
A letter from the Insolvency Practitioner confirming if they do or don't still have an interest in any PPI refunds that may be paid.(Jacqueline Westerman )
A signed confirmation that the IVA has failed (if the IVA has failed)
I have tried to find many of these documents but most of my old paperwork is in bits .
