Hi, sorry for all the questions. I've got my "statement of affairs" pack to fill in, most of which has been done from the information I gave over the phone. Since having it back I've realised that I missed out some information so do I just add in the things I missed myself or phone Accuma and tell them?

Do both to be on the safe side. Are you going to be meeting your IP personally? If so, make sure that these alterations are highlighted during the meetings, and make sure you carefully read through the proposal documents when they are sent to you for signing.
