I have my appointment tomorrow morning, and have filled out the forms in triplicate as requested.

Do I have to take any proof of my debts, ie. all my debts etc, or should I just take the three copies of the forms plus the fee?

You just need to take the forms and the money. I took my driving licence as well in case I was asked for ID but the clerk didn't ask for anything.
