I've just realised that with one of my creditors, my wife was a second card holder. Does this mean that she will also be liable if my IVA is accepted? She never actually used the card in all six years that we had it.

Not unless she is jointly responsible for the account, which appears to be unlikely from reading your post. Just because you have given her the right to have a card, does not make her responsible for paying the account.

Dave was a second card holder on a couple of my accounts and it wasn't even mentioned.
