Am I missing something???

We received our “pack” today from the OR.

It says something about filling in and returning the PRELIMINARY INFORMATION QUESTIONARIE but this appears to be missing from BOTH our packs???

Any recent bankrupts know what this is or what it contains??

I suppose I’ll have to ring the OR office first thing on Monday, our interview is on Friday and they want these documents back by Thursday but now this may not be possible!

I am on a bit of a roller coaster at the moment, one minute I am relieved to be rid our debts but then the next, I am in tears!!!!!!!!!!!!!


What’s wrong with me????

This is obviously an error - ring them on Monday and explain that you have not received the forms.

As melanie says this is propably an error, I dont recall anything of this nature included in my pack. I had information sheets on Bankruptcy, and advice notes on what I needed to return to them eg credit card & Bank account statements, all existing credit and bank cards, check books etc.
Dont worry, ring the OR office Monday, the staff are always very polite and helpful, I am sure they understand that this is for many people a very stressful and confusing time.

Hi Jane, I got all my stuff from the OR's office today. I received a copy of my Bankruptcy Order, and in a separate letter I received:

1) Covering letter telling me my interview date etc, also asking for all my loan agreements/statements and any cards or cheque books.
2) 2 copies of an NTB2 - which basically tells me as a bankrupt what I can and cannot do - sign one and return it.
3) Another copy of my Bankruptcy Order! (yay - anymore copies and I'll be able to send em out as Crimbo cards!)
4) A Tax and National Insurance Disclosure form to send back.
5)An ethnic monitoring form to send back.
6) A comments/suggstions card
7) Leaflets explaining what will happen to my house/bank account/interview with the OR
8) A copy of the Insolvency Charter
and 9) a form authorising the OR to provide copies of statements and questionnaires to the trustee in bankruptcy (ie. IP)

jane you may find this form on the insolvency site, as you i did because i filled my petition in on line, there are several more forms there that appear when you log in once you have been made BR just check it out, they didn't inform me that it was to be done on line but the form was there, and still ring them on monday just to make sure.
what i hadn't realised was i had already filled the form in on the day of BR as i was sent straight to the OR the same day.

I received:

On the covering letter there is a checklist of forms that have to be sent back by Thursday, it covers all the above documents, but the first on the list is a Preliminary information questionnaire PIQDP) – this is what is missing from both packs.


a form authorising the OR to provide copies of statements and questionnaires to the trustee in bankruptcy (ie. IP) - DON’T THINK WE RECEIVED THIS FORM EITHER!

Like Kerri says I found the form on the Insolvency site but I didn't fill it in, and the OR never asked me to.

Try not to worry about it and give them a call tomorrow - if they have missed something out it's not your fault, and the sooner you tell them the sooner they can put your mind at rest!

I've just checked what I received with my bankruptcy pack:

A letter confirming the time and date of the interview;
Details of what to do with my financial records;
Form NTB2 (Notice setting out your duties and responsibilities as an undischarged bankrupt);
Form TNIDIS (Tax and National Insurance Disclosure);
Ethnic Monitoring Form (Why??????);
Comments / Suggestions Card (I never completed this as I forgot);
What will happen to my home? leaflet;
What will happen to my bank account? leaflet;
The Insolvency Charter.

The covering letter asks that you send 12 months of credit card statements, but I've only got the most recent, will this do?

The OR can always request earlier ones if they feel they need them Brassic - this is just for the investigation into your spending during the year leading up to the bankruptcy order, so nothing really to worry about.

I didn't have all of mine so I sent them what I had and nothing was said at the interview. The OR can request them directly from your creditors if they need them, so don't worry.

Ok thanks, is it the same for bank statements? I do paperless statements so I'd have to print all the statements off and I think there's around 200 pages!

I printed off all of my Egg card statements, but I don't know about bank statements. It might be best to give the OR a call and ask them what they want you to do.

I couldn't remember where I'd spent the money either! She was mainly interested in the loans, although she did tell me that if she needs any more information she'll get back to me - fingers crossed I don't hear from her!

If you don't remember what something was for just tell the OR that - they will understand and don't expect you to remember everything.
