This does seem very odd. Once the certificate of completion was issued your partner's name should have come off the register within three months whether successfully completed or failed. It sounds to me that there has been a mix up possibly of names but as his IVA is still live and it is not his fault it should be easily resolved.
I do agree that he should speak directly to the IP as this is potentially very serious and I am sure the IP would want to know. If he cannot get through to the IP send an email make Urgent/Complaint and this should get the required response.

Hi Laura. I am afraid that our old profiles, with contact information, were obliterated along with the old forum. You could include an email address in your signature, using the words "at" and "dot" so that bots don't pick it up, until such time as things change.
