approximately 4 years ago I went to an agency who on my behalf approached my creditors and reduced my monthly payments in total to £250 (total debt around 28k). I have since changed jobs and moved house and can no longer afford to pay this money each month. Would I qualify to start an IVA?
None of my debts are for either income tax or poll tax.

Depending upon the level of yur new disposable income figure, it may be possible to find an IP who would be prepared to take your case on. Talk to advisors such as Thomas Charles who have links with various IPs and will advise you in more depth.

I note that you have been paying into this plan for 4 years now. For the benefit of our other posters could you confirm how much debt you started with when you commenced the payments?
