John, I sent my form to the address on the pdf you sent me - was that right or should I have sent it to you?

I'm being really blonde recently - I can't cope with a house move and an office move in less than a month!

I have also sent my application to the address on the downloaded form! I have included a copy of the email received with my application and sent it Thursday so I guess I'll have to wait to see if it was the correct address.
IVA final payment left the bank on the 26th January 2013...looking forward to a debt free future.

I got a call from bread just now, they are doing my application.The address on the PDF is the right one.
