how can they fail an iva that we never excepted

If you had failed an iva does anyone know how this would show up on a credit report,would it say iva failed or iva completed,if i applied to experian does any one no what i should expect to see

Hi everyone got a copy of my credit file on line yesterday[if i felt bad i feel even worse now]all creditors have placed defaults,i knew this was inevitable,but cant understand it says date iva was excepted,then underneath says iva complete,21/02/07 then iva satisfied 07/03/07 did i miss something,why would they put this when i failed iva.
