Jan is right you are back to square one, but you do have options of a debt management plan or bankruptcy.
This is just my own personal view if I was in your situation, I would contact another professional & discuss what to do next. You have done everything in your power to pay what you can, & it seems you have been very let down by the management of your IVA. I walked into our IVA knowing little about them & if it had not been for this forum, bankruptcy was on the cards for us too. Your not alone & there are others whom have been in this situation, we here to help n support you through this. I hope a solution can be found n keep posting.

Increases enforced like that are reckless to say the least, and I am concerned that no attempt has been made to discuss this with you or to try and vary the terms of the arrangement. I suggest that you arrange to have a chat with your IP directly to get some explanations.
It may be possible for you to present another IVA - if you have the stomach for another five years worth of contributions - this time perhaps with a more helpful IP. However if you have no assets which would be at risk from bankruptcy proceedings, that may be a better bet to simply draw a line under matters and move on.
