The bank will be able to apply charges for unauthorised overdrafts, but they may be sympathetic if you speak to them directly and refund the charges.

There's nothing specific in proposals about this so your IP will have to use their discretion and I think it's only reasonable for the increased income from the o/t to be used.
Just send in a quick letter with your wage slips about this so that it is considered.

Your IP should be dealing with this for you but putting something in writing yourself enclosing a copy of the Chairman's report should help. Ask to speak to a manager about their complaints procedure as well.

This seems very strange - did you say that the bank transferred your call to your IP?
Changes to DD should always be notified in writing - 14 days notice is required if I recall correctly. I would write to your IP with your concerns and asks them respond in writing.
