clearly whatever medication you are on isn't working. Call the doctor and get an emergency appointment. Get them to look at what you are on, and the dosage and see if they can refer you to someone to talk to before the 10th June.

seems to me there is nothing to discuss. Your current job just isn't good for your health or your family. If you have been offered another job with better hours etc then I say just go for it.
Everything else can be worked out after that.
