We have not heard anything back since 4th annual review in September. Now a little worried.

The review must be done within two months of the anniversary so it may be that there is a backlog this year so a short delay. Nothing to be worried about but call your IP anyway for clarification. It may have been overlooked or perhaps not received by the IP.
