Is this a debt incurred since the IVA was set up or was it one missed off by accident.

Depending when the debt first occurred it maybe possible. If the debt was prior to your iva start date then your IP may be able to include it, however if you have incurred the debt after the start of the iva then it may not be so easy or impossible to do.
Talk through the circumstances with your IP and see what they can do to support

Also if this was a pre IVA debt and it is a material amount you MAY have breached your IVA in not disclosing it...! When was debt incurred and how much is it for?
