How do I find out why my IVA has failed without contacting the IP that represented me ?

Good morning, I hope you can answer my question, how do I find out why my IVA failed without contacting the IP that represented me.

If your IVa has failed, then you IP should've sent you some paperwork with all of the details on it. It will be in the Chairmans report.

My IVA is going back to 2008, I have the failed certificate but can't find any chairmans report, I want to know how do I get a copy of this without contacting the IP used.
