We very sadly failed our iva in our 4th year was due for it to end aug this year we failed as we didnt let them know about some extra income we had still totally gutted and feel so foolish about not being upfront about it all spoke about maybe extending the iva and paying back what we earned but they didnt want to know infact all we had was telephone message saying it had been failed rang up to discuss if further to be told that the cerficate would be in the post in a few months time and that we could then re start up another iva coulod someonje please tell mke how long it will be before we receive this cerficate ?? how soon will the creditiors be back in touch ??? and when can we restart another one as we really want to pay off our debits and were so near yet so far due to our own foolisness it just that it failed in nov and we havent hared a single thing since

I would start chatting to another company ( assuming you want a different company -- as the last one didn't seem too helpful come the end) as soon as possible, so they they can get everything in place while waiting for the certificate.
My opinions are merely that .. opinions based on experience. Always seek professional advice.

If they have not issued the certificate then the IVA has not been failed. Get back on to them and demand they try and reinstate the IVA or close the file down. They may still be reclaiming PPI or have a backlog but if you are still on the register the IVA is still live.

I would urge you to take Michael's advice and hopefully it can be resurrected.

This post does highlight the severity of not disclosing all income. Hoping you can move forward with your current providor.
IVA final payment left the bank on the 26th January 2013...looking forward to a debt free future.
